Overview of Penny Lane Centers' Benefits

MEDICAL BENEFITS

PLC offers four (4) medical plan options through Anthem & Kaiser Permanente , including the Kaiser HMO with HRA, the Anthem Narrow Network HMO plan, the Anthem Full Network HMO plan, and the Anthem PPO plan compatible with an HSA.

HEALTH PLAN ACCOUNTS

  • Health Savings Account through HSA Bank - Available to those who are enrolled in the HSA-eligible health plan. An HSA allows you to save pre-tax dollars for current and future healthcare expenses
  • Health Reimbursement Arrangement (HRA) through EBA&M - Available to those who enroll in the HRA-eligible health plan. HRA funds are contributed by Penny Lane and can be used toward your annual deductible and out-of-pocket maximum.
  • Flexible Spending Accounts through Flex Facts - There are two options available: the Health Care Account and Dependent Care Account. These can be used to set aside pre-tax dollars to help pay for IRS-qualified health and/or work-related dependent care expenses.

DENTAL & VISION INSURANCE

Two Dental and one Vision plan options through Guardian - The Managed Dental Guard Network HMO Dental plan, the DG Alliance & Preferred Networks PPO Dental plan, and the VSP Choice Network Vision plan.

DISABILITY PLANS

  • Long Term Disability coverage at no cost through Guardian - The LTD benefit pays up to 60% of your salary for up to 2 years of your own occupation.
  • Voluntary Short Term Disability coverage available to purchase through Guardian - This plan pays up to 30% of your salary up to 24 weeks. It is not integrated with state disability and pays in addition to state disability coverage.

SUPPLEMENTAL INSURANCE

Voluntary medical supplemental benefit options through Guardian - These plans pay benefits directly to you and can help cover potential out-of-pocket costs that are not covered under your medical plan:

  • Accident – Pays benefits for certain off the job accidents
  • Critical Illness – Pays benefits if diagnosed with certain conditions
  • Hospital Indemnity – Pays benefits if admitted to a hospital

LIFE AND AD&D BENEFITS

  • Basic Term Life and Accidental Death & Dismemberment - provided to you at no cost through Guardian.
  • Voluntary Term Life/AD&D - available to purchase for you and your eligible dependents through Guardian. You may elect up to $500,000 for yourself, $500,000 for your spouse/domestic partner, and $10,000 for your child(ren).

RETIREMENT & 401(k)

401(k) savings plan through Transamerica - You will be automatically enrolled the first of the month following 60 days from your date of hire if you are at least 21 years old. You are eligible for employer matching contributions if you have been employed for one year and worked 1,000+ hours during that plan year. Vested contributions are available for those who have worked 2+ years of at least 1,000 hours during each year.

EMPLOYEE ASSISTANCE

  • Employee Assistance Program through Guardian’s WorkLifeMatters program - Paid 100% by Penny Lane and offerscompletely confidential counseling services via in-person visits and phone calls.
  • OneDigital Employee Advocacy - Access to benefits experts who are able to assist with questions regarding benefits, enrollments, claims, and more!

PTO, HOLIDAYS, & PERK PROGRAMS

  • 11 paid holidays for all and 3 weeks (120 hours) of Paid Time Off for years 0-5…and it goes up from there!
