Outreach Triage Team services provide immediate intervention to assist clients that do not presently meet criteria for hospitalization and provides services and supports to avert hospitalization for a period of 30-90 days. We provide outreach and engagement, intensive case management, crisis management, rehabilitation services and collaboration with public and private agencies to coordinate available long-term resources. Clients are referred to the program through the Department of Mental Health (DMH) Access center.
